  • Assists the Director of Care with hiring, supervising, appraising and disciplining of assigned staff. Ensures that all assigned staff receive proper orientation, in-service education and on-the-job training.
  • Acts as a resource for staff seeking to increase knowledge and skill level techniques and processes.
  • In addition to individual convalescent care resident data, calculates and reports convalescent care vacant bed days for monthly electronic submission of the required convalescent care program data to the MOHLTC regional office. Works with CCAC and hospital staff to capture all core data for each convalescent care resident.
  • Manages the utilization, maintenance and purchase of equipment/supplies pertinent to resident care within given budget guidelines for the program.
  • Effectively communicates and interacts with residents, family members, visitors and volunteers in a courteous and professional manner.
  • Responsible for ensuring that the program is operated efficiently and in compliance with The Long Term Care Act and Regulations and other pertinent legislated standards and requirements.
  • Conducts duties in accordance with Chartwell policies and procedures, as well as, government regulations, laws and policies. Works safely to reduce the risk of injury to self, other staff members and residents. Understands and follows all health and safety policies and procedures. Ensures that occupational health and safety policies and procedures are implemented.
  • Promptly reports all actual or potentially hazardous situations. Investigates all on-the-job injuries, accidents and unusual incident reports, and ensures that required forms are completed according to established procedures related to the program.
  • Provides guidance and work direction to staff and students. Monitors staff and uses progressive disciplinary measures if necessary consistent with Chartwell’s policies and procedures and in consultation with the DOC.
  • Responsible for performing all duties in adherence to the guidelines and regulations of the Occupational Health and Safety Act, and the policies and procedures of the residence.
  • Has knowledge and understanding of his/her responsibilities of the Fire Procedure/Disaster Plan of the residence.
  • Assumes responsibility for personal and vocational growth and development through participation in staff development programs, seminars and workshops upon completion of the College of Nurses Reflective Practice.
  • Prepares written performance appraisals for all convalescent care staff on assigned shift.
  • Interviews and hires convalescent care staff in accordance with Chartwell polices.
  • Disciplines staff as needed and in accordance with the residence’s policies and procedures and in consultation with the Director of Care.
  • Assists in the orientation and training of the resident, family, attending staff and students
  • Generates ideas for change, and communicates safety and security procedures. Fosters collaboration and cooperation with the hospital and CCAC so that necessary information is provided promptly, there is time to plan for a smooth transition to the LTCH.
  • Establishes effective working relationships with the residence’s Departments and services.
  • Liaises with various community care programs to ensure necessary programs are available for clients upon discharge.
  • Liaises with attending physicians, laboratories, hospitals and families, as necessary.
  • Maintains effective working relationships with the medical staff, facility staff, residents, families and the general public.

The ideal candidate possesses:

  • Three years’ experience in Nursing Services in a consultative or leadership role is preferred
  • Current Certificate of Competence as a Registered Nurse from the College of Nurses of Ontario.
  • University degree in a health-related field preferred.
