Under the direction of the Registered Staff, the PSW provides direct personal and nursing care to residents so as to fulfil the physical, intellectual, emotional, social and spiritual needs of residents in accordance to home polices, procedure and work routines.

 

RESPONSIBILITIES

 

  • Assists in providing direct care for residents; assists with ambulation, feeding, bathing and grooming.
  • Encourages participation in activity programs by assisting residents to and from activities.
  • Assists with meal service and nourishment delivery to residents and provides feeding assistance as required.
  • Provides emotional and social support to residents/families including palliative care.
  • Adheres to established departmental polices and procedures regarding documentation, quality assurance, fire, safety, environment and infection control.
  • Consults with and informs supervisor of unit activities, needs and matters related to resident care.
  • Participates in care conferences and documentation related to resident care plans.

 

 

QUALIFICATIONS:

 

  • Completed a PSW Program that meets the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association and must be a minimum of 600 hours in duration (including class time and practical experience time)
  • Nursing Experience in long-term care required
  • Knowledge and experience in care planning systems and documentation required
  • Mentally and physically able to perform duties as assigned
  • Ability to communicate positively with other staff, residents and families
  • Demonstrates empathy, concern and interest in providing quality care and service to residents
  • Demonstrates organizational skills and ability to complete assignments in a timely fashion
  • Ability to work independently and as part of a team
  • Ability to provide resident care in accordance with application legislation, job routines, nursing care plans, ability to read and write English and understand verbal and written instructions in English.

Reporting to the Director of Care (DOC), the ADOC is accountable for assisting with the management of all aspects of nursing care within the facility to ensure quality resident care through the effective, efficient delivery of established standards within legislative and budgetary requirements.

 

RESPONSIBILITIES 

  • Assists the DOC in the implementation and evaluation of the nursing department quality improvement initiatives, risk management systems, data collection and analysis, report writing, procedural updates;
  • Takes the lead in the co-ordination and implementation of best practice and risk management initiatives related to resident care (i.e. falls, restraint use, wound management, Infection Control)
  • Effectively manages utilization of nursing supplies
  • Responsible for the High Intensity Needs program
  • Maintains a system of care assessment, planning, implementation and evaluation at the resident and unit program level that maximizes staff resources and contributes to Extendicare Scarborough’s interdisciplinary team approach to resident care
  • Effectively supports the RAI-MDS program
  • Maintains an active presence in the Resident Home Areas mentoring staff while overseeing the provision of quality resident care
  • Initiates investigative and corrective actions related to occupational health and safety; develops and monitors modified work programs for nursing personnel
  • Works collaboratively with the Director of Care on special projects, reviews, and other duties as may be assigned
  • Assumes responsibility for the nursing department in the Director of Care’s absence. 

QUALIFICATIONS 

  • Registered Nurse; BScN preferred
  • Registration with College of Nurses of Ontario
  • Minimum of three years experience in long term care; previous management experience an asset
  • Current knowledge of clinical issues for geriatric residents, infection control, principles of adult education and quality improvement
  • Knowledge of RAI MDS
  • Demonstrated superior leadership and organizational skills; excellent verbal and written communication skills
  • Computer skills: Microsoft Word; Excel, computerized resident care and assessment programs
  • Knowledge of applicable legislation, current nursing and healthcare practices; demonstrates concern and interest in providing quality care and service to residents

Reporting to the Dietary Manager, the Dietary Aide assists with food preparation and serving of meals. Cleans eating, cooking, storage, and serving areas, as well as equipment. Follows established policies, procedures and standards.

 

 

RESPONSIBILITIES

  • Serves food to residents according to individual diets and preferences. Observes time limits for meal serving. Maintains foods at proper serving temperature.
  • Assists Cooks to clean and prepare such uncooked food as salads, vegetables and fruits. Follows basic instructions in preparing such simple foods as breakfast items, basic desserts and garnishes.
  • Clears plates and trays, loads and operates dishwasher. Cleans pots, pans and cooking utensils as per procedure.
  • Participates in proper storage and labeling of dietary supplies and food, including leftovers.
  • Adheres to established departmental policies, procedures regarding quality assurance, fire, safety, environment, and infection control.
  • Assembles nourishment carts. Stocks unit kitchenettes.
  • Participates in facility meetings as assigned. Attends in-service education programs.
  • Other duties as assigned by Supervisor.

 

QUALIFICATIONS

  • Successful completion and/or enrollment in a Food Service Worker Program meeting the requirements of the Ontario Long-Term Care Homes Act required. If enrolled, must complete program within three (3) years of hire date.
  • Previous dietary experience preferred in LTC Home 0-1 year(s).
  • Must be able to stand for long periods of time
  • Ability to read and write English as well as to understand simple verbal and written instructions in English.
  • Ability to work safely, following established safety practices and procedures.
  • Must be able to do moderate to heavy lifting
  • Successful candidate must be available to work days, evenings, weekends and statutory holidays
